St. Charles County Food Inspection Scores: Steak N Shake, Tap House, Black Bear Diner

The St. Charles County Department of Public Health monitors more than 1,300 food service providers in St. Charles County. (The City of St. Peters conducts its own inspections.) Routine inspections are conducted by specialists with the department for proper food safety procedures and are scored by deducting points from a starting score of 100.

The following acronyms are used in violation information: PHF (Potentially Hazardous Food), TCS (Time/Temperature Control for Safety), FCS (Food-contact Surface), NFC (Non-food contact surface), RTE (Read to Eat).

Hot water provided at three compartment sink and handwash sinks. During inspection, technician was present fixing the water heater. Hot water had been out from 3/9/19 to 3/11/19 and had another error code so a technician had to come out today on 3/14/19. It is advised to establishment to discontinue operations when hot water is not available for handwashing or dishwashing.

Torn or broken door seals, hinges etc. (poorly maintained or in disrepair) 2(pnt) – Observed torn gaskets on reach in cooler and walk in cooler in kitchen. Also observed some gaskets with excessive food debris that need to be cleaned.

Non food-grade materials used for food storage. Observed pineapple stored in an open aluminum can in reach in coolers in the kitchen. Pineapple must be transferred to a food grade material container once opened.

Dirty NFCs 2(pnt) – Observed sticky shelving with food residue holding clean pots, pans, and other clean plates. Clean surfaces regularly to avoid build up.

Coats, purses and other personal items stored improperly 2(pnt) – Observed personal items in walk in cooler past expiration and not labeled personal.

Sanitizer strips have been provided to the establishment. Non-smoking signs also provided to establishment. Only things checked for during reinspection.

  • JJ’s Restaurant, 200 Fort Zumwalt Square – score 93

Torn or broken door seals, hinges etc. (poorly maintained or in disrepair) 2(pnt) – Observed torn gaskets on majority of reach in coolers in kitchen including walk in cooler and freezer. This is a repeat violation.

Improper wash water temperature 5(pnt) – Observed no sanitizer feeding through the dishwashing machine.

No Certified Food Protection Manager on site during the inspection. Risk-based 4 facility: At least one MANAGER on duty shall have a nationally recognized certification for food safety. All other MANAGERS shall have a nationally recognized certification for food safety to cover all shifts of operation.

Ready to eat PHFs not dated 5(pnt) – Observed food items past their expiration date in the walk in cooler (Cole Slaw 3/7) and reach in cooler (Milk 3/18). Potentially Hazardous Food items should not exceed their date marking past 7 day preparation or manufactured expiration date.

Food stored on floor or exposed to moisture/contamination 2(pnt) – Observed a box of Brussels sprouts in a cardboard box stored directly on the floor in the walk in cooler. Food items should be placed at least 6 inches above the ground.

Torn or broken door seals, hinges etc. (poorly maintained or in disrepair) 2(pnt) – Observed torn gaskets on multiple reach in coolers in the kitchen. This is a repeat violation.

Vent hood dirty 2(pnt) – Observed vent hoods with excessive grease. These must be cleaned and maintained regularly. Increase frequency of cleaning.

Dirty Non-Food Contract Surfaces. Observed shelving in reach in coolers next to the ice machine to have excessive rust and peeling paint. Replace shelving.

Damaged floor tiles, holes in walls, missing ceiling tiles 2(pnt) – Observed damage floor tiling in the bar area and on stairs to the walk in cooler. Floors must be smooth and easily cleanable.

Observed no recent inspection report report. Report currently posted is from 7/20/2018. Last routine inspection was performed on 11/20/2018 with a score of 86.

Follow-up inspection in 10 business days. “No Smoking within 15 ft. ” window stickers (3) given. One for patio, 2 for front door entrances.

  • Playoffs Sports Bar & Grill, 2922 Highway K Ste. A – score 94

Vent hood dirty 2(pnt) – Observed excessive grease on vent hoods in the kitchen.

Damaged floor tiles, holes in walls, missing ceiling tiles 2(pnt) – Observed peeling paint from floor indicating needing resurfacing in both the kitchen and bar area. Floors must be smooth and easily cleanable.

Previous violations of ice machine and mens restroom have been fixed. (1) “No smoking within 15 ft” sign given to establishment.

Cottleville

Observed No Certified Food Protection Manager on site during inspection. This is a repeat violation.

Employees not washing hands 5(pnt) – Observed employees going from duty to duty not washing their hands (drive through employee not washing their hands in between taking money and food prepping)

No soap at sink 5(pnt) – Observed no soap nor paper towels at the hand sink located across from the grill.

Observed food items hot holding on the table below 135F (Chili 104F; Beans 122F). Items were immediately thrown away by the manager.

PHF?s not held at 41 degrees or below 5(pnt) – Observed food items cold holding on display table and walk in cooler above 41F (Raw beef patty 47F; Mushrooms 62.5F). Items on display table were thrown away by manager.

Torn or broken door seals, hinges etc. (poorly maintained or in disrepair) 2(pnt) – Observed gaskets on the reach in coolers having excessive food debris and potential mold growth. This is a repeat violation.

(1) Observed walk in cooler and freezer having temperatures above normal with equipment on possible de-frost. Establishment must maintain equipment in good repair and fix proper adjustments. (2). Observed up-right freezer located across from the grill area having a damaged floor. This is a repeat violation.

Dirty NFCs 2(pnt) – (1)Observed cabinet under the handsink located across from the grill area having excessive grease. This is a repeat violation. (2) Observed soiled can opener with excessive food debris. Establishment must clean equipment regularly.

Observed urine on the floor located underneath the urinals in the men’s restroom. Observed dirty flooring throughout both the men and women’s restrooms and well as no frequent flushing. Staff should perform regular spot checks on restrooms to maintain facility in good condition.

Repeat violations are subject to 8-304.22 PERMIT REVOCATION. Establishments on a continual basis of repeat violations shall forfeit their health license.
